Insurance Information

FACTS REGARDING INSURANCE

As a courtesy, when scheduling your first dental visit we ask for insurance information so that our insurance coordinator can verify eligibility and get a simple breakdown of your coverage.  If we are not able to verify your insurance at/before your first visit, then payment is due in FULL.

For ALL new patient EMERGENCY visits, payment is always required in FULL at the time of service.  We will gladly bill your insurance and once we receive payment we will reimburse you. Once you’re established with our office, only your estimated co-pay is due at your future appointments.

We are more than happy to process your insurance forms for you.
We ask that you do the following:
  • Bring your complete dental insurance information (including fee schedules) with you and keep us updated on any insurance changes. (We will be happy to make a copy of your benefits.)
  • Be knowledgeable about the benefits, eligibility, co-pays, waiting periods, and yearly maximum and deductibles. Please ask our staff if you have any questions.

Your dental insurance is an agreement between you and your insurance company. We are pleased to process your insurance claims with the understanding that you, the patient, are responsible for any expenses not covered by your present carrier. An ESTIMATE of your dental charges will be given at your request; our office is not responsible for the final accuracy of this estimate.

Please call if you have any questions or concerns regarding your initial visit.
